Outreach and After-Care Program
The primary goal of the Outreach and After-Care Program is to ensure the continued safety and well being of children after they return home from a Casa de Esperanza home. The Outreach and After-Care Program supports families during a child's transition from placement at Casa back to his or her home and monitors the family reunification process for as long as necessary. Caseworkers provide emotional support, encouragement and community referrals to families in an effort to promote the stability of reunification. Occasionally, assistance is available in the form of groceries, diapers and/or formula.
